Column: 'Will it help us win games? Probably not.'

Let others debate whether the jinx even exists.

If I'm Dolphins coach Joe Philbin, I'm blaming "Hard Knocks" for the disastrous couple of months that lie ahead. The rookie coach inherited a team barely better than last season's 6-10 version and someone will have to take the fall once the NFL regular season ends unhappily, again, in Miami. And as all the coaches and quarterbacks who've had to do just that for several decades can attest, it's never too early to start pointing fingers at somebody else.

Admittedly, the curse attached to HBO's popular miniseries doesn't have the history or the cache ? yet ? that the cover of Sports Illustrated magazine and the Madden NFL video game do. The SI cover has enough documented cases for its own Wikipedia page and the game recently was featured in a Forbes magazine piece.

The "Hard Knocks" jinx got off to a similar start in 2001, when the network chose the fresh-off-a-Super-Bowl-win Baltimore Ravens as the subject for the show's debut. Training camp had barely opened when the team learned star running back Jamal Lewis was lost for the season after suffering an ACL tear and MCL sprain in his left knee. That effectively ended any chance the Ravens, who relied heavily on defense and a ground game, would repeat as champions. What the next few episodes of "Hard Knocks" became, beyond a launching pad for the TV career of then-Baltimore coach and current analyst Brian Billick, was the template for how NFL organizations attempt to cope with too much attention.

"Do I think this is going to help us win games?" Philbin asked the day after the first episode was aired, then answered his own question. "Probably not.

"Do I think this is going to cause us to lose games? I don't think so. Again," he added, "my focus is on the development of the team and not necessarily the TV show."

Yet there's a very good reason why teams like the Patriots, Packers, Giants and Steelers haven't volunteered for their turn in the limelight, and why HBO is finding it harder to convince even the needier owners and franchises to play along. The Cowboys tried it twice and both seasons began with promise and ended flat. The 2010 New York Jets, with coach Rex Ryan heading the most entertaining cast of characters on the show, appeared to defy the jinx by overcoming a raft of distractions and going all the way to the AFC title game before bowing out.

Two years later, the Jets are the only team so far this preseason to go without scoring a touchdown. Coincidence? I think not. Especially since the hapless Dolphins, winless in three tries, just happen to be the only team that has yet to even hold the lead in a preseason game. At that rate, there will plenty of blame to go around.

The Dolphins already lost veteran backup QB David Garrard to a knee injury, and wide receiver Chad Johnson (Ochocinco), who was expected to provide a big chunk of the drama, is out of a job following a domestic dispute. Also sent packing via a trade was former No. 1 pick Vontae Davis, whose spot in an already porous secondary will be inherited by a rookie.

Few fans will remember any of this between now and December and will pick on Philbin instead. It's easy to do in part because of his role in "Hard Knocks," where he comes off as a well-intentioned, but ultimately overmatched high school math teacher. He's seen wandering around the complex picking up gum wrappers and upbraiding players for not tying their shoelaces. In one unintentionally comic bit, veterans Reggie Bush, Karlos Dansby and Jake Long turn up in Philbin's office to ask the coach ? on camera ? to communicate more with his players. In the background, we glimpse the occasional sign touting that "Champions Train Here" and "Work Here" and "Play Here" when it hasn't been true in Miami, really, since the great Dolphins teams of early 1970s.

The reason why has little to do with Philbin, though you wouldn't know that from watching the first three episodes. If there's a real villain in this drama, it's owner Stephen Ross, who didn't buy his way into the front office until 2008, but can't resist hitting the reset button on his desk. His first plan was to give old NFL hand Bill Parcells total control. When that didn't produce wins or sell tickets, Ross tried to increase attendance by putting stars in the seats instead of on the field ? selling off bits of ownership to Jennifer Lopez, Marc Anthony, Jimmy Buffett, Gloria Estefan, Fergie and Venus and Serena Williams. That didn't work any better.

One of the reasons Ross has tried desperately to raise the Fins' profile is because of increased competition from Miami sporting rivals, the NBA champion Heat and MLB Marlins, who were the subjects of a "Hard Knocks" copycat called "The Franchise" on Showtime.

Expectations were high at the outset, but despite a spending spree, a new ballpark and the arrival of manager Ozzie Guillen, the Marlins reside in the NL East cellar. Full disclosure, as the Dolphins are about to be reminded, comes with a cost.

Amuay Venezuela Oil Refinery Explosion Kills At Least 39 (VIDEO)

PARAGUANA/CARACAS, Venezuela, Aug 25 (Reuters) - An explosion tore through Venezuela's biggest refinery on Saturday, killing at least 39 people, wounding dozens and halting operations at the facility in the worst accident to hit the OPEC nation's oil industry.
Energy Minister Rafael Ramirez told Reuters no production units at the Amuay refinery were affected and that there were no plans to halt exports, a sign that the incident will likely have little impact on fuel prices.
Photographs taken shortly after the pre-dawn blast showed wrecked vehicles, flattened fences and giant storage tanks buckling and crumpling as flames lit the night sky. A National Guard building in the area was shattered and officials said a 10-year-old child was among the dead.
A gas leak caused the explosion and most of those killed were National Guard troops who were providing security for the 645,000 barrel-per-day (bpd) facility, Ramirez said, adding that the fire was under control.

"There was a National Guard barracks near the explosion. ... The installation was too close to the operations," Ramirez told Reuters in an exclusive telephone interview, adding that production could resume at Amuay within two days at most.
"We need to boost production at other refineries and look for floating storage near the complex," he said.
The incident follows repeated accidents and outages during the last decade across installations run by state oil company PDVSA that have limited output and crimped expansion plans.
Amuay has partially shut operations at least twice this year due to a small fire and the failure of a cooling unit.
Those problems have spurred accusations of inept management by the government of President Hugo Chavez, who is running for re-election on Oct. 7.
Acrimony over the explosion could spill over into an already bitter campaign, but s unlikely to overtake larger political concerns such as crime and the economy.
"I want to convey the deepest pain that I've felt in my heart and soul since I started to get information about this tragedy," Chavez said in phone call to state TV. He declared three days of mourning.

FIRE UNDER CONTROL
Venezuela has traditionally been a big supplier of fuel to the United States and the Caribbean, but refinery shutdowns have become so common that they rarely affect market prices.
Traders told Reuters the docks at the refinery were shut, and tankers were anchored offshore waiting. They said this would cause delays to some of the country's exports.
The explosion broke windows at homes in the area, a peninsula in the Caribbean sea in western Venezuela, as well as at Amuay's main administrative building.
The blast was also felt out at sea in the Paraguana bay, where some crew members on moored tankers were knocked off their feet by the shockwave, one shipping source said.
Ramirez said the fire that started after the explosion had only affected nine storage tanks holding mostly crude oil and some processed fuels including naphtha.
Officials said two tanks were still burning off residual fuel, and a Reuters witness at the scene said large black clouds of smoke still hung above the area.
Ramirez said existing fuel stocks around the country were sufficient to guarantee 10 days of exports and local sales. PDVSA has no plans to invoke force majeure, he said, which lets companies stop shipments due to accidents or extreme weather.
Amuay, together with a neighboring facility, forms part of the Paraguana Refining Center, the second-biggest refinery complex in the world, with an overall capacity of 955,000 bpd.
In 2010, there was a massive fire at a PDVSA fuel terminal on the Caribbean island of Bonaire, then a blaze at a dock at the Paraguana complex that halted shipping for four days.

JOHANNESBURG (AP) ? A hefty hippo chased away from his herd at a South African game reserve has found a refreshing place to relax: the lodge's swimming pool. Now it's stuck there.

The young hippopotamus plopped into the pool on Tuesday at the Monate Conservation Lodge north of Johannesburg. The pool is big enough for the hippo to swim but it's eight feet deep with no steps and "there's no way he can come out," lodge manager Ruby Ferreira told The Associated Press on Thursday.

DUBAI (Reuters) - Protesters pelted police with petrol bombs and stones in clashes that broke out in Bahrain on Tuesday night at the funeral for a teenage demonstrator killed last week in a new bout of unrest in the U.S.-allied Gulf state.

Police arrested eight protesters, the government said. The opposition accused the security forces of provoking the violence by firing tear gas.

Bahrain, where the U.S. Fifth Fleet is based, has been in crisis since a revolt led by majority Shi'ite Muslims began 18 months ago to demand democracy in the Sunni-ruled kingdom.

The government has denounced the protest movement, inspired by uprisings elsewhere in the Arab world, as sectarian and a part of a quest by Shi'ite Iran to dominate the region. Bahraini Shi'ites deny being steered from Tehran.

The violence broke out the funeral of 16-year-old Hussam al-Haddad, who was killed on Friday by police gunfire.

"A group of rioters bombarded police with Molotov cocktails and stones from the roof of a religious centre," a government statement said.

"Another group attempted to block several roads, while still others began an illegal rally on a busy road," it said, adding that eight people had been arrested.

The main opposition Wefaq party said riot police started the violence by firing tear gas at those mourning Haddad.

"Many injuries were reported as the regime forces opened fire at mourners," Wefaq said. "As usual, the regime forces backed the militias who took part in attacking the mourners."

Authorities issue permits for protests but have not granted any since June. Wefaq and witnesses said police set up roadblocks to prevent people attending the funeral, which was held on the island of al-Muharraq.

The disturbances followed the arrest of 11 people on Monday evening during clashes with riot police in the capital Manama and Hamad Town.

Opposition activists said Haddad was also beaten by plainclothes agents. The Interior Ministry said the death resulted from police reacting in self-defense to a petrol bomb attack on a patrol.

Haddad's death was the first in the unrest since April, when a man was found on a rooftop covered in shotgun pellet wounds after clashes with police the night before.

The Shi'ite-led opposition says more than 45 people have been killed in protests since June 2011, when the government lifted martial law it had imposed to help quash the uprising.

They also say hundreds of people have been arrested since the government said in April it would crack down on protesters who skirmished with police. The Interior Ministry has not said how many people are now in detention.

Bahraini authorities say more than 700 police officers have been hurt in clashes and that the police, who have not used live fire, have been exercising restraint. Opposition activists say the police are anything but restrained.

The opposition's central demand is for the elected parliament to have full powers to legislate and form governments.

King Hamad bin Isa Al Khalifa has approved constitutional reforms that give the assembly more powers of scrutiny over ministers and budgets.

The government and political parties have held low-key, inconclusive talks on how to end the turmoil.

Bahrain is an important card in a regional competition for predominance between Iran and U.S.-backed Saudi Arabia.

US horse rescues overwhelmed by abandoned animals | Metro

LINCOLN, Neb. ? The Double R Horse Rescue looks different in drought: There?s little but stubble in the pastures and a chain and padlock on the gate.

The owner began locking up the property last month in an effort to keep people from driving up at night and dropping off horses they can no longer afford to feed.

Horse rescue groups nationwide have been struggling to care for a growing number of animals abandoned since the Great Recession hit more than four years ago, but leaders say their work has become even more difficult and expensive this summer as drought and wildfires burned up pastures and sent hay prices skyrocketing. Many people who held on to their horses in the downturn are now letting them go because they can?t find or afford feed that has more than doubled in price.

Jami Salter is caring for 15 horses at the Double R Horse Rescue in Riverdale, about 150 miles west of Omaha, and she said that?s all she can handle. But she?s still getting three or four calls a week from people asking her to take their horses, and at one point, people were abandoning one or two animals a week.

?People would just drop horses off without asking me,? Salter said. ?Every morning, I went out to water them, and I?d have more horses than the day before.?

Most farmers and ranchers have had trouble growing hay this year because of the drought that stretches from Ohio west to California. Salter said a company that donates to her rescue got 46 bales last year from a 22-acre plot but this year expects only six or seven. Recent wildfires in northern Nebraska have added to the shortage, forcing ranchers to choose between feeding their horses and more profitable cattle.

Salter said she paid roughly $110 last year for a bale that weighed 1,200 to 1,500 pounds. This year, the price is about $260, and it takes seven or eight bales each week to feed the horses in her care.

Gov. Dave Heineman tried to help by allowing farmers to cut hay growing along roads earlier than usual, but Salter said that didn?t do much for her. Her property has seen less than an inch of rain since May, and grass isn?t even growing in roadside ditches.

Roger Kavan also has stopped accepting new horses at the Ruby Ranch Horse Rescue, an 80-acre sanctuary about 90 miles southeast of Denver. He has 20 horses already and said he?s still getting one to three calls a day.

Meanwhile, horse adoptions ? the rescue?s main source of income ? have fallen off as people become more reluctant to take on the expense. Kavan is paying nearly $7,100 per semi load of hay, up from $3,800 last year.

?We could take the easy way out, take them to auction and let the killer-buyers pick them up and wash our hands of it,? Kavan said, referring to people who buy horses in the United States for slaughter overseas. ?But we?ll never do that. We?re going down with the ship.?

Advocates say no one tracks how many horses are abandoned, but the Washington, D.C.-based Unwanted Horse Coalition estimated the number at 170,000 to 180,000 per year in a 2009 report.

The problem appears worse this year based on the number of calls the coalition has received this summer from owners who can?t feed their horses, said Ericka Caslin, the group?s director. Caslin said her group tries to connect horse owners with shelters, but many are already at capacity.

Tony Pecho, president of the Illinois Horse Rescue near Chicago, began the year excited that land purchases and a sharecropping deal with another horse owner had doubled the property he had available to grow hay. But so far, he said, the 70 acres he has this year produced one-fourth of the hay gathered last year.

Pecho said he?s concerned he won?t be able to store enough food to get his 15 horses through the winter, when they won?t be able to graze at all. Meanwhile, the number of calls to his rescue take in abused and neglected animals has increased from four to 20 per month.

?Horses running down the road. Horses let loose in public hunting areas,? he said. ?We?re get calls from people who have had them as pets for five or 10 years, but now they can?t afford them.?

Horse owners said the situation has become desperate because those in trouble have few options, especially since some sanctuaries have stopped taking animals.

?There?s no place to go with a horse you can?t feed,? said Iowa Horse Council President Bill Paynter, of New Virginia, Iowa.

Even horse owners who have the money to buy hay may have a tough time finding it. James Noel of Coatesville, Ind., said he harvested enough to feed his six horses this year, but he didn?t have enough to sell to others. He worries about what some people will do this winter, as supplies wane.

?If people have hay, they?re hanging onto it,? said Noel, president of the Indiana Horse Council. ?It?s taken a toll on all of us, and we?re all sure it?s going to get a lot worse.?

Drs. Cellini and Valentini describe the developments in the management of rectal cancers over the past decades, the results of major trials, and the conclusions members of the medical community have reached concerning these developments and results, which have led to both areas of agreement and controversies. Better imaging, improved surgical techniques, and the integration of radiotherapy alone or with chemotherapy have resulted in marked improvements in loco-regional control but not in survival. Still, in populations in whom there has been a focus on the important components of rectal cancer management, 5-year survival is better in rectal cancer than in colon cancer, which was not the case in the past.[1,2] Dissection outside the embryologic plane surrounding the mesorectum?total mesorectal excision (TME)?is one of many important components of current management.

In order to discuss agreement and controversies, we must agree on what is to be discussed. In recent decades, there has been a substantial increase in what is considered locally advanced rectal cancer; this increase has paralleled the improvements in results. Everyone likely agrees that some rectal cancers can be managed by surgery alone, either TME or a local procedure, with low risk of local failure and good survival. In another group, with slightly more advanced disease, TME will result in an R0 resection but there will be too many local recurrences. In this group, radiotherapy decreases the risk of local recurrence by 50% to 70%, and preoperative therapy is more dose-efficient and less toxic than postoperative therapy.[3] The additional therapy increases morbidity, but treatment is warranted if the risk of local failure is sufficiently high, considering the severity of such failures. In this group, in patients in whom no downsizing or down-staging of the primary tumor is needed, the effects and toxicity of the short-course Swedish 5 ? 5 Gy schedule are well documented; however, long-course chemoradiotherapy (CRT) is used by many clinicians.[4] The additional chemotherapy improves local control but not survival, as has been shown in two large randomized trials.[5,6] A pooled analysis of five trials, including the two randomized trials, has indicated that survival is improved,[7] but the pooling of trials with different designs may introduce bias. In even more advanced tumors, TME will not result in an R0 resection and extended surgery is required. These tumors were previously often described as nonresectable, even if technically they were not. Even if an R0 resection was achieved, local failures were common. In these patients, preoperative CRT to effect down-staging or to at least kill peripherally located tumor cells left behind after surgery is likely unanimously considered standard therapy. The addition of fluorouracil(Drug information on fluorouracil) (5-FU)/leucovorin to 50 Gy significantly improves local control (R0 + no local failure) and disease-free and cancer-specific survival, but not overall survival.[8] In patients who cannot tolerate CRT, a 5 ? 5 Gy schedule with delayed surgery is an effective, low-toxicity alterative, based on recent retrospective studies.[9-11]

What should these three groups be called, and where is the disagreement? The first rectal cancer group, for which surgery alone is sufficient, is usually denoted by the term ?early,? and the second is referred to by most as ?locally advanced??but what about the third group, which requires an entirely different strategy? ?Most locally advanced?? In the Second European Rectal Cancer Consensus Conference, EURECA-CC2, for which Professor Valentini served as chairman,[4,12] the terms ?early? (or ?good?), ?intermediate? (or ?bad?) and ?locally advanced? (or ?ugly?) were used. Cellini and Valentini use these terms once; otherwise, they use ?locally advanced? to refer to the intermediate group. Most phase II/III trials performed over the past decades have chiefly included patients from the intermediate group. Patients with locally advanced (ugly) disease, which constitutes 10% to 15% of primary rectal cancers, are usually excluded, since they would tend to worsen the results of a study.

If we cannot agree on what tumors are included, it is meaningless to discuss agreement and controversies. I believe there is less disagreement than may seem to be the case concerning treatment of rectal cancers if we agree on what is discussed.

In the early group (30% to 40% of tumors), surgery alone is standard therapy for most patients. Which T1 substages can be treated with local surgery is a question open to discussion. Radiotherapy alone (contact therapy or brachytherapy) is used as an alternative[13] but has not gained wide acceptance. Whether a partial mesorectal excision is sufficient in tumors in the upper rectum is debated. Organ preservation may be a consideration for some patients,[14,15] but the morbidity of surgery alone (for some, abdominoperineal resection [APR], and for others, sphincter-preserving surgery) must be weighed against the morbidity of CRT in about 30% of patients, and against the morbidity of both CRT and major surgery in the rest. Most patients included in organ preservation studies belong to this early group.[14,15]

All tumors in the early and intermediate groups benefit from preoperative radiotherapy, which is associated with a relative reduction in local failure rates of 50% to 70%.[3,16] Depending on the absolute risk of recurrence, the number needed to treat to prevent one local recurrence must be balanced against the morbidity that radiotherapy adds to the surgery for all treated. The absolute risk of recurrence is dependent on many factors: tumor height and location, clinical T-stage, N-stage, tumor differentiation, signs of extramural vascular invasion, and the skill of the surgeon. A major disagreement concerning patients in the intermediate group relates to the radiotherapy schedule: short-course radiotherapy with immediate surgery or long-course CRT with 5-FU. Cellini and Valentini give the pros and cons for both approaches. However, it is incorrect to state that long-course CRT increases sphincter preservation; this was a seemingly well-founded hypothesis espoused by many, but it failed in trials.[17] It does not become truer by being repeated.

In the locally advanced (ugly) group, CRT with a fluoropyrimidine is standard treatment?about which there is basically no disagreement. A 5 ? 5 Gy schedule with a delay is an option in patients unfit for CRT. Combinations with other drugs, including targeted drugs, are experimental. Despite apparently favorable results in multiple phase II studies of combinations of drugs (due to the inflation in what is considered locally advanced disease), the phase III trials have failed to show superiority of a combination (such as oxaliplatin(Drug information on oxaliplatin) and a fluoropyrimidine) over a fluoropyrimidine alone, only more toxicity.[18-20] Organ preservation is not relevant in these tumors.[21]

Large randomized trials have provided a solid evidence base that can be used to determine the appropriate treatment for many patients. However, the trials have not determined conclusively which therapy is the best in all clinical situations?which is why we have to extrapolate. Consensus statements or international surveys have been used to overcome lack of knowledge from trials.[4,22] There is a high risk, however, that such statements and surveys may arrive at inappropriate recommendations, since reimbursement (as Cellini and Valentini also mentioned) plays an important role in many treatment decisions, sometimes overriding scientific evidence.

The loco-regional problem in rectal cancer has been more or less resolved in most cases, except for the most locally advanced rectal cancers with extensive lateral growth or growth into the sacral nerves or proximal sacral bone. Fortunately, these constitute only 1% to 2% of newly diagnosed rectal cancers (about 10% of ugly tumors). Integrated boosts should be explored in these cases.[23] In order to improve overall survival, however, other strategies must be explored. The high cell-kill effect of the 5 ? 5 Gy schedule, its brevity and low toxicity, and the possibility it affords of rapidly starting effective combination chemotherapy, also make it at present the optimal regimen for improving survival. This is explored in the phase III RAPIDO trial (Rectal Cancer And Pre-operative Induction Therapy Followed by Dedicated Operation; www.clinicaltrials.gov-NCT01558921) in patients at high risk of local or systemic failure (basically those with ugly disease [cT4, cT3mrf+] and some with bad tumors [EMVI+, N2]). The control group receives CRT, surgery, and adjuvant combination chemotherapy. Rationally designed trials such as the RAPIDO trial may resolve some of the remaining controversies.
