FDA Panel Rejects Avastin for Breast Cancer (HealthDay)
WEDNESDAY, June 29 (HealthDay News) -- The blockbuster cancer drug Avastin got soundly rejected late Wednesday as a treatment for metastatic breast cancer by a U.S. health advisory panel that found the medication was not effective and causes dangerous side effects.
By unanimous vote, the panelists rejected the drug maker's appeal of a U.S. Food and Drug Administration recommendation last December to revoke Avastin's approval for breast cancer. The FDA recommendation cited the medication's poor performance in follow-up studies and its potential for serious side effects.
The drug maker, Genentech, now owned by pharmaceutical giant Roche, was given an unusual two-day hearing this week before the six-member advisory group.
Tuesday's opening-day hearing was dominated by patients and physicians who argued, often emotionally, against the drug's revocation.
"For the patients and their supporters, it was the reality that many of them were alive and well with few symptoms, which they and their physicians attributed to the fact that Avastin had a significant treatment benefit for their breast cancer," blogged Dr. Len Lichtenfeld, Deputy Chief Medical Officer for the American Cancer Society, who attended the hearing.
But later in the day, he added, "the FDA told a different story. The data from clinical trials, after looking at thousands of patients treated with Avastin in addition to more standard chemotherapy, showed no meaningful clinical benefit. At one point, an FDA representative said that they were looking for only one trial that had some sort of benefit .. No such trial emerged, while the side effect risk was considerable."
On Wednesday, according to CNN, Dr. Sandra Horning, one of Genentech's top scientists, cited a February 2008 memo in which the head of the FDA's cancer drugs division made positive comments about the study that he had criticized on Tuesday. Genentech officials also defended E2100, the original study that formed the basis for the FDA's accelerated approval, and said there were no better treatments available for many women, CNN reported.
Lichtenfeld, at the end of the first day of hearings, outlined the dilemma. "Clearly, we are in a place where emotion meets science and the FDA's decision will prove to be a difficult one," he said in his blog.
The advisory panel's vote is non-binding, and FDA commissioner Margaret Hamburg will make a final ruling, the Associated Press reported.
The FDA's unusual step of scheduling another hearing on the drug underscored the difficulty of withdrawing approval of a cancer medication, the AP reported.
"It says to me that either they [the FDA] have gotten a great deal of negative feedback from various quarters, or there's some kind of internal disagreement within the agency," Dr. Gary Lyman, professor at the Duke Cancer Institute in North Carolina, told the news service. Lyman was part of the FDA advisory panel that voted 12-1 last year to revoke Avastin's approval as a breast cancer therapy.
The FDA's December recommendation did not immediately affect breast cancer patients' access to the drug or limit use of Avastin (bevacizumab) for advanced colon, lung, kidney and brain cancer.
Doctors can continue to prescribe Avastin to patients "off-label," as they do other drugs. But it's unlikely that insurance companies would cover off-label use of the drug given its high price tag. Avastin costs more than $8,000 a month, according to published reports.
Avastin was OK'd in 2008 for use in metastatic breast cancer in conjunction with chemotherapy under the FDA's accelerated approval program. Approval was based on a clinical trial of patients with metastatic HER2-negative breast cancer that found a benefit in terms of cancer recurrence -- but not overall survival -- and was contingent on further data to confirm the results.
Three subsequent studies failed to find an overall survival benefit and showed less impressive improvements in survival involving no progression of cancer.

The 7.5ft (2.3m) boa constrictor, called Diva, was last seen in a tank at her owner's home in Broom Crescent on Wednesday morning.
The owner described the snake as "unfriendly" and said it might bite if approached, but it is not venomous.
Suffolk Police has advised people to keep children and pets indoors.
A police spokesman said: "The snake was last fed about three weeks ago and is due a feed.
"Suffolk Police cannot rule out a risk to the public and residents are advised to keep children and pets indoors.
"However, our understanding is that this snake will only feed on small animals such as mice and rats."
It is thought the breeding season could have encouraged the boa constrictor to go out hunting.
The police spokesman added: "She is nocturnal and is likely to hunt at night and will bask in the warm grass or on rocks in the sun during the day.
"It will hide, for example, under sheds when not hunting or warming."
Boa constrictors are commonly found in tropical forests in South America and are from the family of constricting snakes, which kill their prey through asphyxiation.

Reclusive painter keeps Mao spirit alive on Tiananmen (Reuters)
BEIJING (Reuters) ? Reclusive Chinese painter Ge Xiaoguang's art has gazed over one of the world's most famous city squares for decades.
For 30 years, he has painted the portraits of former paramount leader Mao Zedong that look across Beijing's Tiananmen Square.
The giant oil paintings of the "Great Helmsman" have kept watch from the Gate of Heavenly Peace since the Communist Party won the civil war and declared a New China on October 1, 1949.
"I feel honored to have done this all these years. It is a sacred job. The sense of duty is quite strong," Ge said.
The paintings, now made of glass fibre and reinforced plastic, are six meters (20 feet) high and 4.6 meters (15 feet) wide, and weigh up to 1.5 tons.
Ge keeps a low-profile and has refused countless requests for interviews. But he gave Reuters access to his studio near the imposing Forbidden City ahead of Friday's 90th anniversary of the founding of the Chinese Communist Party.
Paintbrushes in hand and standing atop a moving platform, 58-year-old Ge gently smoothes the surface of the canvas, creating an airbrushed effect that lends the chairman a benevolent glow. "The key in the portrait is to depict Chairman Mao's presence. It's really important to manage to show the charisma that he had as a great leader," he said.
Born in Beijing in 1953, Ge learned to paint the large-scale portraits from his predecessor Wang Guodong.
When Wang retired in 1976 after Mao's death, Ge formally became the fourth artist to take up the vaunted position. He now leads a team of artists that depict China's political leaders, from Mao to current president Hu Jintao.
Ge alone paints Mao. Every year since 1977, he has created a new portrait, each one slightly different, which replaces the old one during the night between September 30 and October 1, China's National Day. After decades of referring to archive material and photos, Ge says he knows Mao so well he does not need to spend much time sketching out the portrait.
Mao is still revered in China for uniting a vast country much of which had been divided up by warlords and foreign invaders.
But he is also blamed for the deaths of millions in the famine of the disastrous Great Leap Forward and the chaos of the Cultural Revolution.
The official verdict of his rule is that he was 70 percent good and 30 percent bad.
There has been a resurgence in recent years of nostalgia for the Mao era, with restaurants emblazoned in red featuring the fiery cuisine of his native Hunan, and busts and pins and other knickknacks featuring the chairman have long been popular features in tourist bazaars.
During the 1989 pro-democracy protests centered on Tiananmen Square, three demonstrators splattered Mao's portrait with paint, and later received long jail terms.
Today, the tens of thousands of people that flock to the world's largest square are greeted by Mao's imposing portrait, which currently looks directly at a huge hammer and sickle crafted from flowers for the Party's birthday. Even after 30 years of market reforms have made China the world's second biggest economy, Mao is still an official ideological centerpiece of the Party at 90. Patients plead to keep Avastin for breast cancer (Reuters)
SILVER SPRING, Maryland (Reuters) ? Breast cancer patients testified that Roche Holding AG's Avastin saved their lives as U.S. health officials weighed whether the world's best-selling cancer drug should remain approved for that condition.
At stake in the hearing that began Tuesday is nearly $1 billion in annual Avastin revenues, the hopes of cancer patients and the resolve of the Food and Drug Administration to stick to data-based decisions.
"I am alive due to Avastin," said 66-year-old Patricia Howard. "I'm a wife, mother, sister, aunt and granny. I'm not just a statistic. It's in your hands to make sure I don't become one."
Patient testimony began the unusual two-day meeting to hear Roche's appeal of an FDA proposal in December to withdraw Avastin's approval for breast cancer treatment.
At least 40 patients held signs and chanted outside the FDA building on Tuesday, wearing pink T-shirts with the slogan "We are statistically significant."
But FDA presenters said the agency had to follow the data.
"We cannot permit sponsors to evergreen approval of a drug that has not been shown to be safe and effective," Dr. John Jenkins, director of the FDA's Office of New Drugs, told the appeals hearing.
FDA officials said the agency could review approval of Avastin for breast cancer after new studies, but it would be inappropriate to keep the breast cancer approval as the studies would take at least three years.
For Switzerland-based Roche, the FDA decision could shave almost a billion dollars off Avastin's more than $6 billion in annual sales. Roche shares fell 0.4 percent in European trading on Tuesday.
Roche unit Genentech, which will present its case on Wednesday, said it agreed with the many women and their doctors who testified on Tuesday. "Women with this incurable disease should have the option to choose this medicine," it said in a statement.
Some patients said Genentech should offer free Avastin for those patients that respond well to the drug while it studies which specific patient populations could benefit.
For patients, an FDA rejection of the appeal could mean insurance companies would stop covering the expensive drug for breast cancer, potentially jeopardizing treatment for an estimated 17,000 women currently using the medicine.
Breast cancer is the second-leading type of cancer among women after skin cancer, with 1 in 8 women in the United States expected to develop invasive breast cancer in their lifetime.
In 2010, an estimated 200,000 new cases of invasive breast cancer were diagnosed in the United States, and 40,000 women died from the disease.
The panel will make its recommendation at the end of the hearing on Wednesday, and FDA Commissioner Margaret Hamburg will later make the final decision.
LATER STUDIES LESS PROMISING
Avastin won U.S. clearance for breast cancer in 2008 based on a study showing the drug stalled cancer growth by 5.5 months more when used in combination with standard chemotherapy. As part of an accelerated approval, the FDA required Roche to run follow-up studies to confirm the drug worked.
Later studies found only a one- to three-month delay in breast cancer growth. None of the studies showed Avastin extended the lives of patients with advanced breast cancer.
Some patients also had severe side effects, including holes in the stomach and intestines, severe bleeding and blood clots. The company says the incidence of these serious side effects was just under 3 percent greater for those taking Avastin.
In December, the FDA proposed revoking Avastin's clearance for breast cancer, while keeping the drug on the market for colon, lung, brain and kidney cancers.
Genentech has been fighting the decision, saying the extension of time without cancer growing was meaningful for patients with advanced breast cancer. At that stage, the disease cannot be cured and the goal is to stabilize patients as much as possible.
Some medical experts argue that if the FDA keeps the breast cancer indication solely to placate patient groups, it would undermine the integrity of the accelerated approval process.
"If the FDA demonstrates that it is unable or unwilling to withdraw accelerated approval when the totality of evidence fails to meet its standard for regular approval, such a precedent risks undermining ... the agency's credibility as it seeks to regulate medical products for the public good," said a perspective article published in the New England Journal of Medicine on Monday.
Stripping the breast cancer indication for Avastin would not prevent doctors from using it. Without FDA approval, insurers may refuse to pay Avastin's $8,000-a-month price tag, and Genentech would not be able to promote Avastin for that use.
Most analysts believe the panel will not change its mind, especially as five of the six members voted against Avastin's use in breast cancer at a hearing last July.
Medicare, the U.S. federal health insurer, is likely to cut coverage for Avastin in breast cancer if the FDA revokes that use. That could shrink U.S. sales by half in 2011 to $400 million, said Karen Andersen, analyst at Morningstar.
But she sees a 60 percent chance that private health insurers would still keep coverage for Avastin on an off-label basis.

At a TED talk, the two New York advertising executives learned that captive monkeys understand money, and that when faced with economic games they will behave in similar ways to humans. So if they can cope with money, how would they respond to advertising?
Laurie Santos, the Yale University primatologist who gave the TED talk, studies monkeys as a way of exploring the evolution of the human mind. A partnership was soon born between Santos, and Olwell and Kiehner?s company Proton. The resulting monkey ad campaign was unveiled on Saturday at the Cannes Lions Festival, the creative festival for the advertising industry.
Monkey brands
The objective, says Olwell, is to see if advertising can make brown capuchins change their behaviour. The team will create two brands of food ? the team is considering making two colours of jello ? specifically targeted at brown capuchins, one supported by an ad campaign and the other not.
How do you advertise to monkeys? Easy: create a billboard campaign that hangs outside the monkeys? enclosure.

'Tortoise mafia'
Madagascar's tortoises sell for thousands of dollars on the black market Madagascar's poachers, known in conservation circles as "the tortoise mafia", are increasingly hunting down the Indian Ocean island's reptiles, threatening them with extinction.
The tortoise mafia, who allegedly include corrupt government officials and smuggling syndicates, are satisfying a growing demand locally for tortoise meat and abroad for exotic pets and tortoise shells used in aphrodisiacs.
"Everybody is eating them and everybody is trafficking them and as soon as people are brought to trial, there are mafia organisations who help to get them out," says the head of Madagascar's Alliance of Conservation Groups, Ndranto Razakamanarina.
Another conservationist, Tsilavo Rafeliarisoa, says two poachers were caught last year in southern Madagascar with 50 tortoises.
This was a small breakthrough in efforts to protect the endangered tortoises.
Often, poachers roam villages in groups of up to 100, picking up thousands of tortoises over several weeks.
They are heavily armed, fending off attempts to stop them.
"When a gang of poachers with guns and machetes come and take tortoises, the villagers are defenceless," Mr Rafeliarisoa says.
Tortoise meat is a favourite dish on the island He says with food prices rising, more people are eating tortoise meat.
It has become a favourite snack in southern towns such as Tsiombe and Beloka, even among government officials who ought to be at the forefront of campaigns to save the reptiles from extinction.
"They say: 'Give me the special' - and the special is tortoise meat. It is a huge market," Mr Rafeliarisoa says.
Herilala Randriamahazo of Madagascar's Turtle Survival Alliance says he recently went on a research trip to Tsiombe and Beloka, posing as a tourist to see how common tortoise meat has become on restaurant menus.
To his horror, a bowl of tortoise meat, stewed in tomatoes, garlic and onion, was sold for a mere $2.50 (£1.50).
It was served to him in less than 30 minutes.

"I sent it back. The waiter said he could get me something different, even a live one right away," Mr Randriamahazo says.
He says the streets of Tsiombe and Beloka are littered with tortoise shells - an unfortunate sign of the insatiable appetite people have acquired for them.
Yet, Madagascar's tortoises were once protected by the cultural beliefs of some of the island's communities.
"People respected tortoises. They did not even touch them," Mr Randriamahazo says.
Now, if tortoises do not end up in the rubbish heaps of restaurants, they end up in the suitcases of tortoise smugglers.
Madagascar is known for its rich biodiversity but this has attracted smugglers interested in everything from its precious rosewood to minerals and tortoises - and the famous lemurs.
An alliance of 27 national conservation groups recently accused the government of being complicit in the illegal trade, as it had not cracked down on the "looting and plunder" of natural resources.
A WWF report on Madagascar's biodiversity earlier this month said more than 600 new species had been discovered in the "Treasure Island" over the last 10 years, but many were already endangered.
With only a few hundred of the world's most endangered Ploughshare Tortoises left, hundreds of species are crawling towards extinction behind them.
Tortoise shells litter towns in Madagascar Hasina Randriamanampisoa, of the Durrell Wildlife Conservation Trust, says the smuggling rings are well-organised, selling tortoises on the black market in Asian countries such as Thailand.
Wealthy Asians see tortoises as exotic pets, and are prepared to pay up to $10,000 (£6,250) for them.
Traditional doctors in Asia also buy the shells of baby tortoises, using them in medicine concoctions that reputedly enhance the sexual performance of men.
Conservationists say smugglers pack up to 400 baby tortoises in suitcases, before flying to cities such as Bangkok.
Increasingly, they are also smuggling out adult tortoises to breed in captivity in Asian countries.
Mr Randriamanampisoa says tortoise numbers are rapidly dwindling and they risk extinction over the next decade.
"Even if the poaching stops now, the natural habitat is so vast, there are chances that the females cannot meet the males in the wild to mate and to have babies," he says.
Mr Randriamanampisoa said there four species of tortoise "are endemic to Madagascar, so if they disappear here you will only be able to see them in zoos".
